Take Control of Your Health

and Prevent Type 2 Diabetes

MACT Health Board is proud to introduce Prevent T2, a CDC-recognized lifestyle change program designed to help you prevent or delay type 2 diabetes. If you are prediabetic or at high risk for developing type 2 diabetes, this year-long program provides the education and support you need to make lasting lifestyle changes in a structured, encouraging environment.

This program is offered at no cost to eligible Medicare patients.

The Prevent T2 program is specifically designed for individuals who:

Are 65 years of age or older.

  • Have been diagnosed with prediabetes.

  • Are at high risk for developing type 2 diabetes and want to lower their risk.

  • Are looking for a supportive group environment to learn and adopt healthier habits.

This program is open to both Native and non-native members of our community. No doctor's referral is required to join!Program Structure: A Year of Dedicated Support. Your health journey is a marathon, not a sprint. Our year-long program is structured to give you consistent support as you build and maintain new, healthy habits.

Months 1-4 (Sessions 1-16): Meet once a week for foundational learning and habit-building.

Months 5-6 (Sessions 17-20): Meet every other week to reinforce your progress and navigate challenges.

Months 7-12 (Sessions 21-26): Meet once a month to help you maintain your new lifestyle for the long term.

Program Details

Cost: There is $0 cost to eligible Medicare patients. MACT will bill Medicare directly.
